The following picture shows how to test the voltage. Please contact the seller to tell them your issue.
And could you please tell me how is another extruder motor output voltage?

I tested voltage like shows on picture. Voltage on all four wires is 12,86V, when motors on extruder stopped or moving. Voltage on wires of axes motors varies from 0,17V to 3,65V, when motors on axes stopped or moving.

I am a little confused by you. Is the 12.86V got from PSU?
When the extruder is moving , the voltage between GND and any pin of extruder motor slot is changing instead of a firm value, just like the axes.

Let's check one more thing: Please check if there are 2 groups of 12V DC voltage from PSU when you turn on the printer. Please take a picture, show me how you test the voltage.

If you can not move extruder by LCD (Prepare>move axis>move 1mm>extruder) after raise the temperature, please contact the customer service.
In this post shows how to set your printer in Repetier-Host:

Hello. I replace the control board and extruder motors came alive. But if I press the button "Extrude" in the Repeiter-Host the motor ejects the filament up. If I press "Retrarct" - the motor pulls the filament into extruder. And when I want to print anything the motor ejects the filament up. What I need to do?

I connected all wires like on old control board. I can not to connect wires, because the jack can connect only in one direction. The settings in Repeiter-Host like in instructions. I upload the firmware that you send on my e-mail.
