Plastic Jams in Hotend

Hi. My printer used to work fine. Now when I try to print the extruder jams. It will print fine for the first couple layers then the flow of plastic stops. I can see that the extruder is still running (the gear is turning) but no plastic is coming out of the nozzle... Then I clean the nozzle out, extrude some plastic manually, try again.... and the same thing happens over and over. What has happened to my printer? I've tried different rolls of plastic thinking that maybe it was a problem with moisture. Even bought some high quality plastic brand new. Nozzle temperature seems to be consistent... If I try to push plastic through the nozzle by hand it will go but after a jam I have to push very hard, even though the extruder is up to temp.

Help Please!

This can happen if the teflon tube feeding the filament is not pushed all the way into the hot end
