possible under extrusion

I feel this is under extrusion but I cant fix it. I am using pla and in the pic the dog on the left is the original that I printed when I got my printer and it was at 205 degrees. I have tried
1. cleaning my nozzle and even replaced it
2. making sure tube is not obstructed
3. feed gear is plenty tight
4. increasing temp and increasing flow
I haven't used my printer in several weeks and It was last working fine and haven't changed anything. now any outer walls are not smooth and infill is very thin to the point it just crumbles very easily. I have increase the temp to 225 and increased the flow and have gotten better results but nowhere close to before. Could this be a hot end problem or could my temps be reading wrong.
